Overview
The Progresa Home Care Management System (HCMS) is a comprehensive solution that allows you to cohesively manage the financial and clinical information of your Hospice and Palliative Care patients from admission through discharge and the family bereavement process. HCMS is designed to support multiple levels of care in diverse settings including home, hospital and nursing homes to improve the services delivered to the patient and their family and friends. Improve Care Coordination The Clinical System for the Point of Care (POC) and Office provides a multidisciplinary approach to documentation and coordination of care for each discipline including nursing, physical therapy, occupational therapy, aide, speech therapy, medical social work, nutrition, pharmacy, chaplain, and volunteers. In the POC product, the clinician can view the entire clinical record across all disciplines assigned to the patient for care planning, medication and pain management, infusion therapy, and initiation of treatments and procedures. The multidisciplinary team is able to enhance the quality of services and patient and family satisfaction by coordinating care and case management. Clinicians are able to complete all documentation at patient's side and then easily transfer it back to the office to be accessed by the team, billing and payroll. By following these processes we reduce unnecessary travel and administration time and ensure that the focus is on the provision of care to the patient. Further, as patients can be assigned remotely and they are taken under care immediately. Once the patient is under the clinician's care, the clinician has the flexibility to schedule and maintain encounters, as well as coordinating visits with other team members.
