Care Paths
Overview

Care paths are standard components in the Progresa Health Systems software. Because a care path is derived from accepted industry practices for specific diseases groups or functional deficits, it ensures that the level of care is consistent across similar patient populations. As a result, patient outcomes of care can be monitored, quality and consistency can be maintained, and care outcomes can be measured to facilitate continuous improvement and development of the best clinical practices.

  • Derived from industry standard care plans and designed to manage the major care components of specific disease management
  • Assists the clinician in educating the patient/caregiver(s) in relationship to the skills and knowledge necessary to manage a disease process independently
  • Ensures consistency between clinicians and enables measurement of the results of care

By improving clinical practice, an agency can demonstrate expertise in addressing specific patient populations and improving health status. Care paths also benefit agencies directly by helping to maximize services and control costs.

Care paths help to identify variance in care needs among specific patient populations so that appropriate proactive changes can be made in the delivery of care.

Why Care Paths?

The healthcare industry continues to move toward improved disease process management in an effort to measure outcomes of care, enhance the quality of care, manage services more efficiently, and ultimately achieve more cost-effective healthcare. Other healthcare issues are driving the need for a proactive approach to managing disease, including resource limitations in nursing, the increasing elderly population, and overall client satisfaction. Furthermore, the benefits of care paths are significant in the prospective payment system (PPS) environment where financial resources are limited, and quality and cost-effective care are essential.
